I am sure you can get them from any medical supply store, but also within is a software program on line that you can enter your blood pressure reading and it will chart it for you etc. (see blow link). Or you can just type them up next to dates and times, etc.

The easiest way to do to be exact print out the chars yourself. Type them up and print them.
Why would you need one? It would be comfortable to produce one on your computer anyway.
